I agree to this Coaching Agreement and Client Bill of Rights.
I understand that these coaching services include problem-solving training, modeling success, intention and goal-setting, and exploring options for moving towards my goals. The services Connie Jo Holmes provides are considered non-therapeutic and no form of diagnosis, psychotherapy, or counseling will be provided. If either I, or my coach, determines a need for mental health services, I agree to ask for a referral to a licensed healthcare provider or agree to accept the referral provided to me by my coach.
I understand that coaching is a partnership that requires collaboration, honesty, and effort on my part to manifest the desired outcomes. The purpose of our sessions is to engage in creating options both personally and professionally and to learn new skills that will help me acquire the desired outcomes of this service. My coach will support me in creating strategy, plans, and actions to reach my goals. I understand that Connie has several different modalities and certifications that can assist me in reaching my goals, including hypnosis and health coaching. I understand that Connie will make recommendations on the best form of modalities to use, when necessary.
I understand that it is my responsibility to implement strategy and plans, and that I will request of my coach any support or guidance needed to assist me in this outcome.
I understand that I may terminate or discontinue the coaching agreement at any time. For any monies paid in advance of services received, upon request, those will be returned to you. For any monies paid and services were provided, those monies will not be returned.
CLIENT’S RESPONSIBILITY/IMPORTANT THINGS FOR YOU TO KNOW:
In requesting these services, I understand that to be successful I must be entirely willing to:
1. As with all choices you make as an adult, you are fully responsible for your well-being during our sessions as well as our time working together, including your choices and decisions. My services, coaching and hypnotherapy, are not a substitute for counseling and should not be used in place of therapy. My services focus on positive personal growth with goal setting that can lead to positive outcomes when the client remains focused and consistent. Referrals will be given, if needed.
2. Coaching and/or hypnotherapy is a comprehensive process that may involve all areas of your life, including work, finances, health, relationships, education, recreation, and general outlook on life. Deciding how to handle these issues and implement your choices is exclusively your responsibility. Together we will facilitate the creation and development of personal, professional or business goals and to develop and carry out a strategy and plan for achieving those goals.
3. Coaching and/or hypnotherapy is not to be used as a substitute for professional advice by legal, medical, financial, business, spiritual, or other qualified professionals. You will seek independent professional guidance when appropriate. All decisions and any actions regarding these areas are your sole responsibility.
4. It is important for you to remember that your commitment and follow-through are essential to the success of our time together. This is why I can’t guarantee any results to be achieved because it is up to you and the effort you put into the process. Although there are no guarantees on the outcomes from coaching and/or hypnotherapy, most people report significant progress on their goals and understanding of themselves.
5. Be honest and open with me always. You can say anything to me, positive or negative. This includes honesty in your responses to me and letting me know if something makes you uncomfortable or you don’t want to respond to a question. If at any time you experience discomfort during one of our sessions or are unsatisfied with my approach, please bring that to my attention. We are working as a team to grow your life positively, and together we’ll make the necessary adjustments to keep the process moving forward.
6. I will challenge you. At times I may challenge you or ask tough questions to help move the process forward and overcome perceived limitations. If I make a request, you may accept, reject, or alter the request. My job is to help you move forward, which can be uncomfortable for you at times.
7. We will speak on a regular basis, usually weekly or twice per month. Before the end of each session, we will set a time for our next session. If you prefer, we can select a time that will be the same from week to week. Our time together begins when you are scheduled to call or arrive, not when you actually call or arrive. If you are late, the session will still end at the originally planned time. I require a 24-hour cancellation notice for non-emergency reasons, or you agree to forfeit that session. Please know that I spend time in preparation for our appointments and make every session customized to you and your desired outcomes.
MY RESPONSIBILITY TO YOU:
In order to support you in deriving maximum benefits from our scheduled time together, I agree to:
1. I will be fully present and available for you during our scheduled time. I will come to every session fully prepared for our time together, whether it is reading a new study, identifying a useful exercise for our time together, or creating a one-of-a-kind homework assignment for you. Because of the preparation I do in advance of every session with you, canceling appointments with little to no notice is not an effective use of my time.
2. I will be unconditionally constructive and supportive always. I will always speak the truth to you as I see it. You may not always agree with me, and I count on you to tell me when those times may happen.
3. Our calls and visits and information you share with me are kept confidential to the extent allowed by law. This includes prohibition against disclosing that you are a client, without your written permission. However, as a client, you are free to tell anyone you choose about the benefits you are getting from our working relationship. At no time will I use any information you share with me for my own personal benefit without your written permission.
4. While confidentiality is crucial to the quality of our working relationship, if you share with me that you are engaged in illegal activity or intend harm to yourself or others, I reserve the right to disclose that information to the appropriate authorities.
5. Occasionally, certain topics and information may be shared anonymously with other coaching or hypnotherapy professionals for training or consultation purposes. Your identity or details that could identify you will never be disclosed without your express written permission.
6. Inform you immediately if, in my judgment, you would be better served by another professional or an alternative/complementary means of reaching your goals.
7. As part of my professional development and certification, I may be required to submit your name, contact information, and number of hours to a certifying body (the topics and details of our sessions will NEVER be revealed). I assume I have your permission to provide this information, unless I have something in writing asking me not to.
8. I am available to you between sessions. I encourage you to call or text or email when you have a challenge or have a success to share. If we are working within a long-term monthly approach, we will be working with one another in between our scheduled appointment times and this is part of the service provided to you.

Education and Training:
I was trained and certified in hypnotism at the National Guild of Hypnotists, Inc. I am a Certified Member of the National Guild of Hypnotists and I do annual continuing education to maintain my training at a high level. The National Guild of Hypnotists is the oldest and largest hypnotism organization in the world and its certification is the most widely recognized credential for the professional practice of the hypnotic arts. I am also a member of the International Certification Board of Coaches and Hypnotists, as well as a certified trainer. I also was trained and certified in life coaching at the Life Purpose Institute. I gained my health coaching certification as an independent certified Optavia Coach in partnership with the MacDonald Center for Obesity Prevention and Education & M. Louise Fitzpatrick College of Nursing at Villanova University.
Notice:
AS THE STATE OF WISCONSIN HAS NOT ADOPTED EDUCATIONAL AND TRAINING STANDARDS FOR THE PRACTICE OF HYPNOTISM, THIS STATEMENT OF CREDENTIALS IS FOR INFORMATIONAL PURPOSES ONLY. Hypnotism is a self-regulating profession and its practitioners are not licensed by state governments. I am not a physician nor a licensed health care provider and may not provide a medical diagnosis nor recommend discontinuance of medically prescribed treatments. If a client desires a diagnosis or any other type of treatment from a different practitioner, the client may seek such services at any time. In the event my services are terminated by a client, the client has a right to coordinated transfer of services to another practitioner. A client has a right to refuse hypnotism services at any time. A client has a right to be free of physical, verbal or sexual abuse. A client has a right to know the expected duration of sessions, and may assert any right without retaliation.
Redress:
I am a certified member of the National Guild of Hypnotists, and practice in accordance with its Code of Ethics and Standards. If you have a complaint about my services or behavior that I cannot resolve for you personally, you may contact the National Guild of Hypnotists at P.O. Box 308, Merrimack, NH 03054-0308, (603) 429-9438, to seek redress. Every member of the National Guild of Hypnotists provides this information to every client to ensure ethical practice.
Schedule and Fees:
All fees, our meeting schedule, and meeting durations have been explained to me in advance. This agreement will commence on the date it is accepted herein and submitted electronically. I suggest working with packages because multiple sessions are highly recommended for optimal change. I do require payment in full at the time of signing the Agreement, unless other arrangement have been made. The purpose behind this request is that the client is much more likely to complete all sessions and stay committed to the program. You will be given 30-days notice of any change in fees. Payment can be made via cash, check (made payable to BU2BFULL, Inc.), credit card, Zelle, PayPal or Venmo. I do require a minimum of 24-hour notice for cancellation or reschedule. If it is less than 24-hours you forfeit the appointment as well as the payment for that appointment.
Confidentiality:
I will not release any information to anyone without a written authorization from you, except as provided for by law. You have a right to be allowed access to my written record about you. I understand that my coach will keep business and personal information acquired about me confidential and will only disclose information to a third party about the content of the coaching session when written permission is provided by the client in advance. Clients should know that there are limits to confidentiality and that courts will likely not protect the confidentiality of this service, and in the unlikely event information or records are required by a court, information about these services may be subpoenaed and requested information will be provided.
Insurance:
I suggest you think of my services as something that you will pay for personally. That will both protect your privacy and help you value the work you are doing more. In general, insurance companies do not cover hypnotic services, and I caution you not to expect them to do so.
My Approach:
My philosophy is to understand my client’s model of the world and to work with you through that lens. Connected to this, I treat each client as an individual and customize my approach to help ensure my client receives optimum results.
Limited Liability:
Except as expressly provided in this Agreement, the Coach makes no guarantees, representations or warranties of any kind or nature, express or implied with respect to the coaching services negotiated, agreed upon and rendered. In no event shall the Coach be liable to the Client for any indirect, consequential, or special damages. Not withstanding any damages that the Client may incur, the Coach’s entire liability under this Agreement and the Client’s exclusive remedy, shall be limited to the amount actually paid by the Client to the Coach under this Agreement for all coaching services rendered through and including the termination date.
Dispute Resolution:
If a dispute arises out of this Agreement that cannot be resolved by mutual consent, the Client and Coach agree to attempt to mediate in good faith for up to 45 days after notice given. If this dispute is not resolved, and in the event of legal action, the prevailing party shall be entitled to recover attorney’s fees and court costs from the other party.
Applicable Law:
This Agreement shall be governed and construed in accordance with laws of the State of Wisconsin, without giving effect to any conflicts of laws provision.
